Why Wellness Plans?
Wellness Plans provide preventive veterinary services for one year, with the cost divided into 12 affordable monthly payments. You can upgrade your plan as your pet ages.
All plans include:
- Preventive care examinations and core vaccines.
- A copay examination fee on scheduled visits, if additional examinations are needed.
- Additional discounts on eligible items not included in the plan.

Puppy and Kitten Plans

Help your pet get started on the right paw. At Lake Pointe Veterinary Hospital, we offer an array of Wellness Plan options to choose from and plans can be personalized to meet your young pet’s needs.
What’s even better, Wellness Plans are good for your pet and your wallet.
Our Puppy and Kitten Plans
Kitten or Puppy includes all the preventive care examinations, core vaccines, and basic laboratory tests necessary to help protect pets against illness and disease.
Kitten or Puppy & Spay/Neuter is designed for comprehensive care during the first year of a pet’s life and also includes our routine spay or neuter surgery package.
Routine spay or neuter packages include:
- Morning of surgery, vitals are checked by the veterinarian
- General anesthesia
- Anesthesia monitoring
- IV catheter & fluids
- Pre-surgical blood work
- All pain management and antibiotic injections
- Surgical procedure

Kitten or Puppy
- Preventive care examination (up to 3)
- $35.00 examination copay fee (unlimited)
- Core vaccine series (based on age, breed, and risk)
- Kittens: FVRCP (x3), FeLV (x2), Rabies (x1)
- Puppies: DHPP/DHLPP (x3), Bordetella (x1), Rabies (x1)
- Toe nail trim (unlimited with examination or copay)
- Feline triple test (FIV, FeLV, and heartworm) (kittens)
- Fecal (up to 2)
Kitten or Puppy & Spay/Neuter
- Preventive care examination (up to 3)
- $35.00 examination copay fee (unlimited)
- Core vaccine series (based on age, breed, and risk)
- Kittens: FVRCP (x3), FeLV (x2), Rabies (x1)
- Puppies: DHPP/DHLPP (x3), Bordetella (x1), Rabies (x1)
- Toe nail trim (unlimited with examination or copay)
- Feline triple test (FIV, FeLV, and heartworm) (kittens)
- Fecal (up to 2)
- Urinalysis
- Routine spay or neuter surgery package
Restrictions
- Plan is non-transferrable and only applies to client and pet enrolled.
- Plan cannot be applied to past visits.
- An appointment must be scheduled for services to be used (cannot be used with emergencies or walk-ins).
- Additional discounts apply to eligible non-plan items only and cannot be used with other promotions and/or discounts.
- Toe nail trims must be used with a comprehensive exam or copay.
- Additional costs may apply to nail trims in the event the patient needs sedation.
- If the patient is in heat, pregnant, or cryptorchid at the time of surgery, additional fees will apply.
- All technician appointments will be charged a technician fee and a biohazard fee that are not covered by the wellness plan.

Every pet has a unique set of needs. At Lake Pointe Veterinary Hospital, we offer an array of Wellness Plan options to choose from, and plans can be personalized to meet your pet’s needs.
What’s even better, Wellness Plans are good for your pet and your wallet.
Our Adult Plans
Adult includes biannual preventive care examinations, core vaccine boosters and basic laboratory tests necessary to help protect pets against illness and disease.
Adult & Dental includes all the benefits of our Adult Plan along with our routine dental cleaning package (extractions not included).
Routine dental cleaning packages include:
- Morning of surgery, vitals are checked by the veterinarian
- General anesthesia
- Anesthesia monitoring
- IV catheter & fluids
- Blood work (included in plan)
- Digital dental radiographs
- Scale and polish of all teeth

- $35.00 examination copay fee (unlimited)
- Core vaccine boosters (based on age, breed, and risk)
- Cats: FVRCP (x1), FeLV (x1), Rabies (x1)
- Dogs: DHPP/DHLPP (x1), Bordetella (x1), Rabies (x1)
- Toe nail trim (unlimited with examination or copay)
- Heartworm test (dogs)

- Wellness blood profile (complete blood count, chemistry screen, and urinalysis)
